Merchandise will be invoiced at prices in effect on the date the order is shipped. PAYMENT TERMS: NET 30 DAYS. In order to expedite and facilitate the payment of your account, we maintain a Lock Box System. Please forward your cheques to the nearest Post Office Box in your area as indicated below. DELIVERY TERMS: All orders (not applicable to controlled drugs and narcotics) totalling $5000.00 or over will be shipped prepaid via the seller's choice of carrier. 1. Total orders less than $5000.00 (excluding taxes) will not be accepted. No minimum required for hospitals. 2. Bristol-Myers Squibb Canada products may be combined with those of ConvaTec, Linson Pharma, Oncology and Virology and Westwood-Squibb in the calculation of the total amount of the order. All controlled drugs and narcotics orders will be shipped by ALS Services. Total controlled drugs and narcotics orders less than $2,000 (excluding taxes) will not be accepted. Controlled drugs and narcotics can not be combined with any other BMS products to total $2,000. GOODS LOST OR DAMAGED IN TRANSIT: Bristol-Myers Squibb Canada's responsibility ceases upon delivery of the goods to the customer (CIP Destination). Goods which are damaged, defaced, or lost in transit must be brought to the attention of Bristol-Myers Squibb Canada within 21 days following the shipment date. RETURNED GOODS - General Merchandise returned up to 12 months after expiration date, will now be credited, to the most complete quarter for liquids or on a per pill basis, on the original purchase price (invoice / purchase order number reference) or the current price, whichever is lower. 1. (MTI) 2121 McCowan Road Toronto,  Ontario M1S 3Y6 Note: All returns to USF Processors Canada Inc. shall be issued by Bristol-Myers Squibb Canada at 95% of the credit value. 3. Credit will not be issued for: a) merchandise returned in other than the original container, or returned after one year of expiration date. b) products that have been involved in a fire, flood, bankruptcy sale or returned by other than the original purchaser. Representatives are not authorized to recuperate creditable products for return; this merchandise must be returned by the customer. They can, however, assist the customer in preparing the goods for shipment ensuring that the correct address labels are affixed. RETURNED GOODS - Controlled Drugs C / Narcotics N Controlled drugs and narcotics may be returned to the original point of purchase only once the customer has requested and received written authorization from the qualified person in charge.
