Return Policy

General Conditions

While it is the policy of GlaxoSmithKline Inc. to fully cooperate with any customers who need to return product, some general rules must be set.

Product returns (including Overstock/Unexpired Products) are to be shipped FRIEGHT PREPAID to Med-Turn International Inc. (MTI). A Med-Turn International Inc. (MTI) Return Forms must be filled out completely and included with your returns. Return Forms are available directly from Med-Turn International Inc. (MTI) at 416-298-1234 or 1-888-784-2323.

Narcotics and Controlled Drugs will require authorization from Med-Turn International Inc. (MTI) or an authorized wholesaler.

Product returns may also be shipped to an authorized wholesaler however these returns will be subject to the wholesaler's return policy.

For Hospital customers utilizing the Canadian Pharmaceutical Distribution Network (CPDN), returns may be processed through CPDN. WE ask that you please complete a CPDN "Request for Credit Form".

Returns, which are ineligible for credit, will not be returned to the customer.

No credit exists until a credit note is issued by GlaxoSmithKline Inc., and no deduction, offset, or any adjustment to invoice may be made by the customer except on the basis of a credit note.

GlaxoSmithKline Inc. is not responsible for credits associated with goods lost or damaged in transit.

Product Complaints

Where goods are returned by a consumer on account of an alleged defect, the retailer should immediately notify GSK Customer Service at 1 800 387-7374 of the complaint and return goods directly to GlaxoSmithKline Inc. Full credit will be issued by GlaxoSmithKline Inc.

Expired/Outdated Products

No Authorization is required to return expired products sold by GlaxoSmithKline Inc. that are acceptable under the guidelines in this Credit and Returned Goods Policy. Products are eligible for credit at any time up to 12 months after a product's date of expiry or discontinuation date. Such products should be returned FREIGHT PREPAID to Med-Turn International Inc. (MTI) or an authorized wholesaler.

Overstocks/Unexpired Products

Med-Turn International Inc. (MTI) is authorized to accept overstock or unexpired product returns for GlaxoSmithKline Inc.

Prior authorization in writing is required from Customer Service to return any fully saleable product or unexpired products that are in excess of customer's needs.

Authorized overstock or unexpired product returns will be eligible for partial credit calculated at current list price minus a 15% restocking fee.

No credit will be issued for unauthorized overstock and unexpired product returns.

Incorrect Shipments

When incorrect goods are received, the customer must notify Customer Service within 3 working days of receipt and return the incorrect items freight collect for a full credit within 20 days.

Acceptable Returns

GlaxoSmithKline Inc. will accept for credit:

• Products returned up to 12 months after product's date of expiry or discontinuation date.
• Part packages must be 1/4 full to be processed for credit.

Non-Acceptable Returns

GlaxoSmithKline Inc. will not give credit for:

• Unexpired products
• Products returned after 12 months from product's date of expiry or discontinuation date.
• Any products broken or damaged due to negligence and improper storage.
• Products involved in a fire or flood.
• Products purchased on a non-returnable basis.
• Unexpired products which have been repackaged, overlabelled, pricestickered or reconstituted.
• Products such as items purchased from a bankruptcy sale or going out of business sale.
• Products not originally sold in Canada by GlaxoSmithKline Inc. (or predecessor companies).

Value of Credit

Credit will be based upon 95% of current list price (or current contract price for products purchased on contract) for expired products or discontinued products up to 12 months from the product's date of expiry or discontinuation date.

For opened packages, credit will be given for quantity pro-rated to closest quarter package on expired products only. Example: When 40 pills are returned from a 100 pill bottle, credit will be issued for 1/2 the bottle. NO CREDIT will be given on part bottles for unexpired product.

Shipping errors or damaged goods (except goods damaged or lost in transit) and Product Complains will receive credit at full invoice price.

Authorized overstock or unexpired product returns will be eligible for partial credit calculated at current list price minus a 15% restocking fee.

ClavulinŽ

All returns of ClavulinŽ must be routed to Wyeth-Ayerst Distribution Centres and credit will be issued by Wyeth-Ayerst in accordance with their policy.
