| ELECTRONIC USER AGREEMENT AND LICENSE
While the return form and its instructions have
been provided in a bilingual format, by signing this User Agreement
you have accepted that all information on the WEB Site, as well
as this User Agreement, will be provided in English.
Access Codes:
In order to become a member of the WEB Site you
will have selected a unique User ID and password ("access codes").
You accept all responsibility for the security of your access codes,
which are for your personal use and you are not permitted to disclose
them to fellow employees or to a third party. You have the ability
to modify your password at any time by calling Customer Service
at MTI (416) 298-1234. Please notify MTI immediately if you wish
to terminate your User ID and password or have these access codes
reissued.
Termination and Modification:
MTI may elect to update, modify, change or terminate
all or any part of the functionality available through the WEB Site,
including modification or termination of the ability to make inquiries
at any time in the future. Any amendments of modifications may be
provided to you through an on-line notice. You agree that the WEB
Site contains confidential and proprietary information of MTI and/or
its clients as well as copyrighted information which is not to be
disclosed to a third party.
Your Obligations:
You agree that you will not use the WEB Site to
falsify user information provided to MTI, to engage in conduct that
would constitute a criminal offence. You will not in any way exploit
information obtained through the WEB Site which is protected by
copyright or proprietary rights without permission from MTI.
NCD Access codes (if Applicable):
Along with your user ID and password, a unique
6 digit personal identification number (PIN) will be assigned to
you. You accept all responsibility for the security of your PIN
number, which is provided for your personal use only and is not
to be disclosed to a third party. Narcotic and Controlled drugs
must still be sent to MTI under the Chain of Signature (COS) process
stipulated by federal regulations. MTI cannot be held responsible
for any misdirected parcels or those lost in the mail.
